Cut a piece of BTP 9 X 8 3/4. Score the 9" length at 4 1/8 and 8 1/4.
Fold on score lines and adhere the edge together at the 1/2" fold line.
Score your 9" tube at 4 inches. Fold like a card leaving the openings on the left. Punch a 1/2 circle with a 2" punch on both openings. These are pockets 1 and 2.
Cut another BTP 3 X 11 3/4. Score at 3/4, 3 3/4, and 7 3/4.
Cut from same BTP as above 3 X 4 1/4. Attach this piece at the 1/2" fold to the end of the piece above. Be sure to attach it on the end of the 4" section.
You should now have a piece that is 3 X 16. Fold on the fold lines in a zigzag forming a "W." This will be the inside pockets 3 and 4 that will hold two tags.
Attach your pocket insert 1/2" from the fold on the inside of your card.
Cut a piece of BTP 3 3/4 square and attach to the front pocket of your card. Punch the 1/2 circle to match the first one.
Cut another 3 3/4 square BTP and adhere inside the front pocket and to the front of the back pocket. Punch the 1/2 circle to match the other one. Cut a 3 3/4 X 2 BTP. Adhere to the inside of the back pocket.
Cut a 3 3/4 X 4 3/4 BTP and attach to the back.Make your 4 tags and insert in the pockets.
